COSTCO質量審核流程：The audit is not pass/fail. The audit is for continuous improvement and the score to is to help guide the factory to make the changes. 審核不是通過/不通過。 審核的目的是為了持續(xù)改善， 分數(shù)是為了幫助引導工廠做一些改變。
AUDIT PROCESS(for audit result only desk review)審核流程（初審+文審）：
*CS schedule after receiving GMP Audit Request Form from Costco within 30days. 收到Costco 的GMP Audit Request Form 后， SGS CS （客服）開始排期( 從收到Request 算起，要求30 天內完成審核)。
*Factory fill materials ， pay audit fee and inform SGS CS on the wrong information on GMP request form, SGS CS need tell Costco before audit. 工廠填寫資料，付款，并通知SGS CS 在GMP Audit Request Form 上的錯誤信息 ，SGS CS 需要在審核之前通知Costco。
*Auditor audit factory and submit report 審核員審核工廠并提交審核報告
TR team release report to Costco and payer (within 5 Working Days after on site audit) 技術評審組把報告發(fā)給Costco 和付款方( 審核后的5 個工作日內)
*DR send email to factory for corrective evidence （3 chances for 30 days after on site audit ） 文審組發(fā)郵件給工廠提交整改證據(jù)( 工廠在審核后的30 天內有3 次提交機會)。
*DR will review and approve the corrective evidence within 30 days, then send to Costco for further approval after all findings are closed, If not, factory will not be active in Costco system. 文審組會在30 天內評估和批準整改證據(jù)，全部關閉后，提交給Costco 做最終批準, 否則，工廠在Costco 的系統(tǒng)里還是未被激活的。
*Auditor will be arranged for partial re-audit after desktop review based on factory audit result which need partial re-audit （within 1-3 months after initial on site audit ）. 根據(jù)工廠的審核結果需要現(xiàn)場跟進的，則文審后（ 如果30 天完成不了文審，可以不關閉所有問題點，直接實施跟進審核 ）， 審核員會被安排實施現(xiàn)場跟進審核( 現(xiàn)場審核后的1-3 個月內完成跟進) 。
*Auditor conduct partial re-audit and submit report 審核員實施跟進審核（如果工廠審核當天可以完成整改，則NC 也可關閉，最終工廠不能關閉的NC 才保留下來），并提交審核報告。
*TR team release report to Costco and payer (within 5 Working Days after onsite audit) 技術評審組把報告發(fā)給Costco 和付款方( 審核后的5 個工作日內)。
*DR send email to factory if there are findings exist 如果還有不符合項，文審組發(fā)郵件給工廠提交繼續(xù)提交整改證據(jù)。
*DR will review and approve the corrective evidence within 30 days, then send to Costco for further approval after all findings are closed, If not, factory will not be active in Costco system. 文審組會在30 天內評估和批準整改證據(jù)，全部關閉后，提交給Costco 做最終批準, 否則， 工廠在Costco 的系統(tǒng)里還是未被激活的。
*Auditor will be arranged for full re-audit after desktop review based on factory audit result which need full re-audit （within 1-3 months after initial on site audit ）. 根據(jù)工廠的審核結果需要重審的，則文審后審核員會被安排實施重審( 現(xiàn)場審核后的1-3 個月內完成跟進) 。
*Auditor conduct full re-audit and submit report 審核員實施重審并提交審核報告。
*TR team release report to Costco and payer (within 5 Working Days after onsite audit) 技術評審組把報告發(fā)給Costco 和付款方( 審核后的5 個工作日內)。
*DR send email to factory if there are findings exist 如果還有不符合項，文審組發(fā)郵件給工廠提交繼續(xù)提交整改證據(jù)。
*DR will review and approve the corrective evidence within 30 days, then send to Costco for further approval after all findings are closed, If not, factory will not be active in Costco system. 文審組會在30 天內評估和批準整改證據(jù)，全部關閉后，提交給Costco 做最終批準, 否則， 工廠在Costco 的系統(tǒng)里還是未被激活的。
Reminding提醒：
1. If factory delay for any longer than required amount of time based on the auditscore to conduct any sort of CAP/ partial re-audit, a full re-audit maybe required. 基于審核的分數(shù)， 如果遲于所要求的完成時間實施CAP review或partial re-audit(現(xiàn)場跟進)，可能會被要求安排重審。
2. DR will review and approve the corrective evidence within 30 days, thensend to Costco for further approval after all findings are closed, If not, factorywill not be active in Costco system. 全部不符合項關閉后，SGS DR 才可以提交給Costco 做最終批準, 否則， 工廠在Costco的系統(tǒng)里還是未被激活的。
